Poudre High School Seniors have the opportunity to select a current PHS staff member to hand them their diploma at graduation. This person is someone who has played a significant role in their high school experience. Please use the form linked below to select three staff members. They must be a current PHS staff member. We have provided a list by department to help assist you. If there is a staff member that is not listed, please write their name into one of your top three choices.
